The first mistake is the unit error: 0.2 mg is 200 mcg, but a researcher who reads “0.2” and draws 0.2 mL from a 2 mg/mL vial has injected 400 mcg. Doses are written in mcg and units, never in unlabeled decimals. The second is the concentration error, when 10 units from a 5 mg/mL vial (500 mcg) is treated as if it came from the 2 mg/mL vial (200 mcg).
