tirzepatide dosage in published clinical research starts at 2.5 mg once weekly and climbs in 2.5 mg steps every four weeks to a maintenance dose of 5, 10 or 15 mg, with the 15 mg arm of SURMOUNT-1 producing 20.9% mean weight loss over 72 weeks.
No other dosing variable matters as much as that four-week step: it is what allowed 2,539 trial participants to reach doses that would be intolerable if given on day one.
Tirzepatide is a 39-amino-acid dual agonist of the GIP and GLP-1 receptors, carrying a C20 fatty diacid chain that binds albumin and stretches its half-life to about five days. That half-life dictates everything below: once-weekly injections, a 96-hour missed-dose window, and an escalation slow enough for plasma levels to reach steady state. This guide covers the trial doses, the escalation calendar, the reconstitution math for every vial size, and the mistakes that derail protocols.
PSPeptides supplies research-grade tirzepatide in seven vial sizes from 10 mg to 60 mg, each with a lot-numbered COA. For the overview of the compound itself, the tirzepatide research guide covers mechanism and trial history; this page is the dedicated tirzepatide dosage reference that guide points to.

Tirzepatide Dosage at a Glance
The table condenses the tirzepatide dosage numbers that recur across the SURMOUNT, SURPASS and pharmacokinetic literature. Every figure is sourced in the sections that follow and assumes subcutaneous injection of a reconstituted lyophilized vial rather than a prefilled pen.

Why a Dual GIP/GLP-1 Agonist Is Dosed Once a Week
Tirzepatide binds both the glucose-dependent insulinotropic polypeptide (GIP) receptor and the GLP-1 receptor, with an affinity profile that favors the GIP receptor. The peptide backbone is 39 residues, roughly 4,814 Da, and a C20 fatty diacid is attached at lysine 20. That diacid binds serum albumin, which shields the molecule from renal filtration and enzymatic breakdown.
Albumin binding is why the elimination half-life sits at about five days, or roughly 117 hours. Peak plasma concentration arrives 8 to 72 hours after a subcutaneous injection, absolute bioavailability is around 80%, and clearance is a sluggish 0.061 L/h. Weekly injections therefore produce smooth, overlapping exposure rather than the sharp peaks and troughs of a peptide dosed daily.
Steady state takes about four weeks, which explains the four-week rule in every tirzepatide dosage schedule: stepping up earlier means judging tolerability against a plasma level that is still rising. Our peptide half-life chart puts tirzepatide’s 117 hours next to semaglutide’s roughly seven days and the two-hour half-life of short GH secretagogues, which is why their dosing calendars look nothing alike.
What SURMOUNT and SURPASS Established About Dose and Response
Tirzepatide has more human dose-response data than any other peptide sold for research. The SURMOUNT program tested it for weight regulation and SURPASS for glycemic control, and both series randomized participants to fixed maintenance doses reached through identical 2.5 mg steps. The table summarizes the trials that define the tirzepatide dosage range.
| Trial (year) | Population | Dose arms | Duration | Key outcome |
|---|---|---|---|---|
| SURMOUNT-1 (2022) | 2,539 adults with obesity, no diabetes; mean 104.8 kg | 5, 10, 15 mg vs placebo | 72 weeks | Weight −15.0% / −19.5% / −20.9% vs −3.1% |
| SURMOUNT-2 (2023) | 938 adults with type 2 diabetes and obesity | 10, 15 mg vs placebo | 72 weeks | Weight −12.8% / −14.7% vs about −3% |
| SURPASS-2 (2021) | 1,879 adults with type 2 diabetes on metformin | 5, 10, 15 mg vs semaglutide 1 mg | 40 weeks | HbA1c −2.01 / −2.24 / −2.30 vs −1.86 points |
| SURMOUNT-3 (2023) | 579 adults after a 12-week lifestyle lead-in | Maximum tolerated dose vs placebo | 72 weeks | Weight −18.4% vs +2.5% |
| SURMOUNT-4 (2024) | 670 adults after 36 weeks of open-label escalation | Continue vs switch to placebo | 52 weeks | Further −5.5% vs +14.0% regain |
Pivotal tirzepatide dosage trials with sample sizes, dose arms and primary outcomes.

The tirzepatide dosage step from 5 mg to 10 mg bought an extra 4.5 percentage points of weight loss in SURMOUNT-1, while the step from 10 mg to 15 mg added only 1.4 points. Responder rates tell the same story: roughly 85%, 89% and 91% of participants on 5, 10 and 15 mg lost at least 5% of body weight, against 35% on placebo, and about 30%, 50% and 57% lost 20% or more.
In SURMOUNT-2 the same doses produced smaller losses in participants with type 2 diabetes, a pattern seen across the incretin class. SURPASS-2 showed that all three maintenance doses beat semaglutide 1 mg on HbA1c, and body weight fell 7.6, 9.3 and 11.2 kg on 5, 10 and 15 mg against 5.7 kg on semaglutide.
SURMOUNT-4 answered the question researchers ask most about duration. After 36 weeks of open-label escalation to the maximum tolerated dose, participants had lost 20.9%; those randomized to continue lost a further 5.5% over the next 52 weeks, while those switched to placebo regained 14.0%. Dose maintenance, not just dose magnitude, determines the outcome.
The Escalation Schedule: 2.5 mg to 15 mg in Four-Week Steps
The tirzepatide dosing schedule used in every pivotal trial is the one printed in the prescribing information on DailyMed: 2.5 mg once weekly for four weeks, then increments of 2.5 mg at intervals of no less than four weeks. The 2.5 mg dose exists for gut adaptation and has no maintenance role; a protocol that stays there is underdosing by design.
| Weeks | Weekly dose | Role in the protocol | Semaglutide step over the same weeks |
|---|---|---|---|
| 1–4 | 2.5 mg | Initiation; GI adaptation only | 0.25 mg |
| 5–8 | 5 mg | First maintenance option | 0.5 mg |
| 9–12 | 7.5 mg | Transition step | 1.0 mg |
| 13–16 | 10 mg | Second maintenance option | 1.7 mg |
| 17–20 | 12.5 mg | Transition step | 2.4 mg (semaglutide maximum) |
| 21 onward | 15 mg | Maximum maintenance dose | 2.4 mg continued |
Tirzepatide titration by week as used in SURMOUNT-1, with the semaglutide 2.4 mg escalation shown for comparison.
Three rules govern the tirzepatide dosing schedule. A step is never taken earlier than four weeks. A step is postponed, not skipped, when nausea or vomiting is still active at the end of a block; the protocol holds the current dose for another four weeks. And a step is never larger than 2.5 mg, because the SURMOUNT data exist only for that increment.
Against the other incretin peptides, tirzepatide titration is the middle path. Semaglutide climbs from 0.25 mg to 2.4 mg over 16 weeks, as detailed in our semaglutide dosage and reconstitution guide. Retatrutide’s Phase 2 program ran arms from 1 mg to 12 mg with four-week steps; the retatrutide dosage guide walks through that calendar, and the retatrutide vs tirzepatide comparison sets the two escalation curves side by side.
Tirzepatide Dosage by Research Application
Adiposity and energy-balance models
Weight-regulation protocols set tirzepatide dosage at 10 mg or 15 mg because that is where SURMOUNT-1 found the dose-response curve flattening. A subject who tolerates 10 mg but struggles at 12.5 mg is held at 10 mg, which still delivered 19.5% mean loss; pushing to 15 mg adds about 1.4 points at the cost of higher discontinuation.
Glycemic and insulin-sensitivity studies
Glucose-focused work uses the full 5 to 15 mg range, because even 5 mg lowered HbA1c by 2.01 points in SURPASS-2. Protocols that combine tirzepatide with insulin or sulfonylureas reduce the partner agent, since the prescribing information flags hypoglycemia in exactly that combination. Fasting glucose falls within two to four weeks, well before meaningful weight change appears.
Maintenance and withdrawal designs
SURMOUNT-4 is the template: escalate to the maximum tolerated dose over 36 weeks, then hold. Researchers modeling dose reduction rather than abrupt withdrawal step down by 2.5 mg per four-week block, mirroring the escalation in reverse, although no published trial has validated a taper. The 14.0% regain on placebo within a year shows what abrupt cessation produces.
Body-composition and lean-mass work
In the SURMOUNT-1 DXA substudy, fat mass fell by roughly a third and lean mass by roughly a tenth, so about a quarter of the weight lost was lean tissue. Protocols that care about muscle pair tirzepatide with resistance-loading models and, in some designs, GH secretagogues; our guide to muscle loss on GLP-1 peptides covers those combinations in depth.

Reconstituting Tirzepatide Vials: Water Volumes, Concentrations and Syringe Units
This is the section that decides whether a tirzepatide dosage protocol delivers 2.5 mg or 25 mg. There is no single concentration across the range. Every tirzepatide vial is a 3 mL vial, so the more peptide it holds, the more concentrated the solution has to be: a 10 mg vial in 2 mL gives 5 mg/mL, while a 60 mg vial, which takes at most 2.5 mL, gives 24 mg/mL. The table gives the bacteriostatic water volume and the resulting concentration for every vial size.
| Vial size | Bacteriostatic water added | Concentration | Volume for 2.5 mg | 2.5 mg doses per vial |
|---|---|---|---|---|
| 10 mg | 2 mL | 5 mg/mL (5,000 mcg/mL) | 0.5 mL = 50 units | 4 |
| 15 mg | 2.5 mL | 6 mg/mL | 0.42 mL = 42 units | 6 |
| 20 mg | 2 mL | 10 mg/mL | 0.25 mL = 25 units | 8 |
| 30 mg | 2.5 mL | 12 mg/mL | 0.21 mL = 21 units | 12 |
| 40 mg | 2.5 mL | 16 mg/mL | 0.16 mL = 16 units | 16 |
| 50 mg | 2.5 mL | 20 mg/mL | 0.125 mL = 12.5 units | 20 |
| 60 mg | 2.5 mL | 24 mg/mL (24,000 mcg/mL) | 0.10 mL = 10 units | 24 |
Tirzepatide reconstitution volumes by vial size. Every vial is 3 mL, so concentration rises with vial size — from 5 mg/mL for the 10 mg vial to 24 mg/mL for the 60 mg.
The vial lineup maps onto the escalation calendar: 10 mg is weeks 1 to 4 at 2.5 mg, 20 mg is weeks 5 to 8 at 5 mg, 30 mg is weeks 9 to 12 at 7.5 mg, 40 mg is weeks 13 to 16 at 10 mg, 50 mg is weeks 17 to 20 at 12.5 mg, and each 60 mg vial is one four-week block of 15 mg. Every vial empties inside its 28-day window.
Above 5 mg, a 5 mg/mL solution would need an injection volume over 1 mL, more than a standard insulin syringe holds. The larger vials cannot reach 5 mg/mL anyway — a 3 mL vial caps the water — so they arrive more concentrated, which reduces the draw. Where a dose still exceeds 100 units, split it across two sites.
| Weekly dose | At 5 mg/mL (10 mg vial in 2 mL) | Doses per 10 mg vial | Doses per 15 mg vial | Doses per 30 mg vial |
|---|---|---|---|---|
| 1.25 mg | 0.25 mL = 25 units | 8 | 12 | 24 |
| 2.5 mg | 0.5 mL = 50 units | 4 | 6 | 12 |
| 5 mg | 1.0 mL = 100 units | 2 | 3 | 6 |
| 7.5 mg | 1.5 mL (two 75-unit injections) | 1 | 2 | 4 |
| 10 mg | 2.0 mL (two 100-unit injections) | 1 | 1 | 3 |
| 12.5 mg | More than the 10 mg vial holds | 0 | 1 | 2 |
| 15 mg | More than the 10 mg vial holds | 0 | 1 | 2 |
Tirzepatide units per dose at 5 mg/mL (10 mg vial in 2 mL), with full doses per vial for the 10, 15 and 30 mg sizes.
For doses of 7.5 mg or more, the 30 mg vial reconstituted with 2.5 mL, the most its 3 mL vial takes, gives 12 mg/mL: 7.5 mg becomes 0.63 mL (63 units), 10 mg 0.83 mL (83 units), and 15 mg 1.25 mL split across two sites. With 2 mL the same vial gives 15 mg/mL, where 15 mg is 1.0 mL (100 units), 10 mg is 0.67 mL (67 units) and 5 mg is 0.33 mL (33 units).
Let the lyophilized vial and the bacteriostatic water reach room temperature, swab both stoppers, and draw the target water volume. Direct the stream against the glass wall rather than onto the powder; tirzepatide dissolves within a minute or two with gentle swirling and is never shaken, because the acylated peptide foams and can aggregate.
The finished solution should be clear and colorless. Draw each dose with a fresh U-100 insulin syringe, reading the scale exactly as the tables show: 1 mL is 100 units, 0.1 mL is 10 units, and 1 mg is 1,000 mcg (µg). When a vial size, water volume or dose changes, the PSPeptides peptide calculator recomputes the tirzepatide units per dose instantly, and our how to reconstitute peptides walkthrough covers technique in detail.
Storage before and after reconstitution
Lyophilized tirzepatide is stable for months at 2 to 8 °C and longer frozen at −20 °C, sealed and away from light. Once bacteriostatic water is added, the 0.9% benzyl alcohol preservative supports repeated withdrawals for up to 28 days at 2 to 8 °C. Reconstituted solution is never frozen; freeze-thaw damages the peptide and expanding ice can crack the vial.
The pharmaceutical pens offer a reference point: the label allows up to 21 days unrefrigerated below 30 °C. A research vial punctured weekly through a tirzepatide dosage block is less protected than a sealed pen, so refrigeration between draws is the standard. Our peptide storage guide covers the full temperature-and-time matrix.

Injection Timing, Site Rotation, Missed Doses and Protocol Length
Tirzepatide is injected subcutaneously once every seven days, at any hour and independent of meals; a five-day half-life makes tirzepatide dosage timing within a day irrelevant. The trials rotated among the abdomen, the front of the thigh and the back of the upper arm, avoiding any bruised or indurated site. Intramuscular delivery changes absorption and has no trial data behind it.
The injection day can move if at least 72 hours separate the last dose from the next. A missed dose is given within four days (96 hours) of its scheduled time; beyond that it is skipped and the next dose is taken on the usual day, never doubled. Two doses inside 72 hours stack plasma levels and produce exactly the nausea the tirzepatide dosage calendar is designed to avoid.
Protocol length in the trials ran from 40 weeks (SURPASS-2) to 72 weeks (SURMOUNT-1 and SURMOUNT-2), and SURMOUNT-4 extended total exposure to 88 weeks. Because escalation alone consumes 20 weeks, tirzepatide dosage is not cycled in the eight-to-twelve-week pattern used for short peptides. Washout follows the five-half-life rule: about 25 days for plasma concentration to fall below 3% of steady state.
Side Effects Rise With Each Dose Step: The SURMOUNT-1 Numbers
Gastrointestinal effects are the main dose-limiting factor, and SURMOUNT-1 reported them by tirzepatide dosage arm across 72 weeks. Most events were mild to moderate, clustered during escalation, and faded once a dose had been held for a few weeks. The percentages below are the reason the schedule holds each step for four weeks.
| Adverse event over 72 weeks | Placebo | 5 mg | 10 mg | 15 mg |
|---|---|---|---|---|
| Nausea | 9.5% | 24.6% | 33.3% | 31.0% |
| Diarrhea | 7.3% | 18.7% | 21.2% | 23.0% |
| Constipation | 5.8% | 16.8% | 17.1% | 11.7% |
| Vomiting | 1.7% | 8.3% | 10.7% | 12.2% |
| Discontinuation due to adverse events | 2.6% | 4.3% | 7.1% | 6.2% |
Approximate SURMOUNT-1 adverse-event rates by tirzepatide dosage arm, as reported by Jastreboff et al. (2022).
The pattern is not perfectly linear: nausea peaked in the 10 mg arm and vomiting in the 15 mg arm, while constipation was lowest at 15 mg. What rises consistently is discontinuation, which is why a protocol that reaches its endpoint at 10 mg should not escalate further just because the schedule permits it.
Beyond GI effects, the prescribing information carries a boxed warning for thyroid C-cell tumors observed in rodents, plus warnings for pancreatitis, gallbladder disease, dehydration-related kidney injury, hypoglycemia alongside insulin secretagogues, and diabetic retinopathy complications. Delayed gastric emptying also lowers absorption of oral drugs for four weeks after initiation and after each dose increase.

What Researchers Pair With Tirzepatide
Tirzepatide is rarely stacked with another incretin agonist, because the receptors overlap and the GI effects add up. Retatrutide is studied as an alternative rather than a partner: its added glucagon-receptor activity produced 24.2% loss at 12 mg in Phase 2, and the three-way semaglutide vs retatrutide vs tirzepatide analysis compares dose, potency and escalation length.
The combination that appears most in research designs is tirzepatide with a GH secretagogue, aimed at the lean-mass fraction of weight loss. A CJC-1295 and ipamorelin blend dosed at 100 to 300 mcg nightly acts on a separate receptor system, so the two run independently; the secretagogue follows its own daily schedule while tirzepatide stays weekly.
BPC-157 appears in a smaller set of protocols for gastrointestinal tolerability, given its gut-protective data in rodent models, and is dosed at 250 to 500 mcg daily in that context. None of these pairings changes the tirzepatide dosage itself; the incretin schedule stays exactly as the escalation table describes.
Dosing Errors That Wreck a Tirzepatide Protocol
The most expensive tirzepatide dosage error is confusing milligrams with milliliters. A note that says “0.5” without a unit can mean 0.5 mg (20 units at 2.5 mg/mL) or 0.5 mL (2.5 mg at 5 mg/mL), a five-fold difference. Every dose record should carry mg, mL and units together, and every vial should be labeled with its concentration and reconstitution date.

Second is skipping the escalation. Starting at 5 mg or 7.5 mg because 2.5 mg “does nothing” trades a few weeks of patience for a nausea and vomiting rate the trials never measured, since no SURMOUNT arm started above 2.5 mg. Third is stepping up every two weeks; steady state is not reached until week four, so a two-week step means dosing on top of a still-rising plasma level.
Fourth is doubling after a missed week, which the 96-hour rule exists to prevent. Fifth is carrying the units from one vial over to another: 50 units is 2.5 mg from a 5 mg/mL vial, 3.0 mg from a 6 mg/mL vial, and 12 mg from a 24 mg/mL vial. Sixth is drawing 2 mL into a 1 mL syringe by topping up, which delivers an unrecorded volume. Seventh is leaving the reconstituted vial at room temperature or freezing it.
Every one of these is caught by a ten-second check; our walkthrough on how to use the peptide dosage calculator shows the workflow. In our experience handling vials, the label is the step people skip, and it is the one that prevents the five-fold error.
Why 99%+ Purity Matters More Than the Math
A perfect tirzepatide dosage calculation is worthless if the vial holds 7 mg of peptide instead of 10, or a truncated sequence that binds neither receptor. PSPeptides tirzepatide is manufactured in US facilities operating under cGMP guidelines, and every lot is tested by independent accredited laboratories: HPLC for purity, mass spectrometry for identity, plus endotoxin, sterility, heavy-metal and fentanyl screening. The lot-numbered COA is posted at pspeptides.com/certifications.
Vials run from 10 mg at $54.99 to 60 mg at $199.99, and because the sizes match the four-week escalation blocks, a full 20-week titration uses one 10 mg, 20 mg, 30 mg, 40 mg and 50 mg vial, 150 mg in total. Orders ship free with same-day processing, checkout accepts cards, Afterpay and Klarna, and the 99%+ purity standard comes with a free replacement if any lot falls below it.

Frequently Asked Questions
How many units is 2.5 mg of tirzepatide?
It depends on the vial. A 10 mg vial in 2 mL gives 5 mg/mL, so 2.5 mg is 0.5 mL — 50 units on a 100-unit insulin syringe. A 20 mg vial in 2 mL gives 10 mg/mL, so the same dose is 0.25 mL or 25 units. A 30 mg vial in 2.5 mL gives 12 mg/mL, making it 0.21 mL or 21 units. The units depend entirely on how much water went into the vial.
How long does a 10 mg tirzepatide vial last?
At a tirzepatide dosage of 2.5 mg, a 10 mg vial holds four doses, exactly the four-week initiation block, or two doses at 5 mg. Reconstituted with 2 mL of bacteriostatic water it stays inside the 28-day use window for the whole block. At maintenance, the 40 mg vial is four weeks at 10 mg and the 60 mg vial four weeks at 15 mg.
What happens if a weekly tirzepatide dose is missed?
Published tirzepatide dosage guidance allows the missed dose within 96 hours of the scheduled time, after which it is skipped and the normal schedule resumes. The five-day half-life means plasma concentration falls by roughly 40% over four days, so one late dose barely dents exposure. The next dose is never doubled.
Can tirzepatide be split into two smaller doses per week?
Some research protocols divide a weekly dose into two injections 3.5 days apart to blunt the post-injection peak, which the pharmacokinetics make reasonable because exposure is governed by the weekly total. No trial has tested split dosing, so the SURMOUNT efficacy and side-effect numbers apply only to once-weekly injection.
How does tirzepatide dosage compare with semaglutide and retatrutide?
Tirzepatide runs 2.5 to 15 mg weekly, semaglutide 0.25 to 2.4 mg, and retatrutide 1 to 12 mg in Phase 2, but the milligram figures are not interchangeable because each peptide has its own potency. The tirzepatide dosing schedule takes 20 weeks to reach 15 mg, semaglutide takes 16 weeks to reach 2.4 mg, and all three are once weekly.
